two form builder problems

Hi there -

I've got a few forms with required elements up, but even when the proper information is entered, the user can't submit it without receiving this error:

You must enter data in required field.

It works just fine when I uncheck required.


My second issue is that after someone submits a form, with all the information entered correctly, the e-mail that gets sent only has a few bits of information in it. This has only happened on one specific form, and the only thing I can think of is that there are two different boxes with the same label, though the names are different. (i.e. I have address - mail and address - bill, but both the labels simply read Address).

Any ideas?
