Yeah, I just made my own. You can do it either coding through the vivvo application which I did at first, or develop your own in Flash (my second choice).

The flash way looked a lot better, plus, it gives it a more "up-to-date" feel.

Well, I can tell you what I used to make the first one (in flash) I had "CoffeeCup's Form Generator" but if your a little more savvy you can easily make a form in HTML and have the outputs send emails to a destination email...

(Trust me, you'll end up using the form builder by CoffeeCup more than once!)

Touche' you definitely could. However, for more a more customizable solution thats ((also free))! I'd do my above suggestions
